Do you have a plan in place to control and influence difficult medical decisions that might take place in the future? This 4-part course, led by Ramon Neufeld, M.D. (Retired Family Physician), will guide you through the process of creating your plan to assure that difficult medical decisions related to your care will reflect and honor your personal values, wishes, and preferences. The sessions cover: 1) the need to have a plan; 2) identifying values and beliefs to determine preferences and wishes; 3) the tools used to create your plan; and 4) the steps for implementing your plan. This plan is a gift to you and your family and assures that your personal wishes will be honored and respected during times of medical uncertainty. Recommendation: Enroll a family member to attend with you. This is a great opportunity to have these important discussions. The class is offered in a 4-week class or a 2-week class.
